blow up engine and transmission

Just curious, what causes the blow up of engine and transmission on an automatic and manual car?

I'll give you an example using a manual car....Suppose you're driving along at 90mph in 3rd gear, which is at 6000rpms (just a guess), and you want to upshift to 4th. However, instead of 4th, you hit 2nd, and when you let out the clutch, the engine will have to go WAY over redline to try and spin with the wheels. Since it's not designed for that, you'll blow the engine. It will literally break apart.

I think I was reading about these cases on some RSX board I came across. Apparently, their gates weren't "well-defined", so a lot of people kept ending up in 2nd, rather than the intended 4th.
