Brake pad and Rotor recommendation for Maxima 2013 SV?
Hello,
My 2013 Maxima SV vibrate on steering wheel in speeds above 40 mph when braking.
1) Do I need to replace Brake pads and Rotors?
2) Is there anything else that I need to replace if your answer is yes?
3) Could I replace pads and rotors by myself?
4) What is the option (options) in terms of quality, price?

Unless you have judder when braking then it probably is not a problem with the pads or rotors. It sounds like a balance problem or bent wheel.
I had a problem with my '09 SV and the only way the dealer was able to cure it was to remove the clamp-on weights from the wheels and replace them with stick-on weights.
There are several things that could cause the vibration but I would start with a wheel balancing.

I checked with mechanic and got following feedbacks:
It says following items should be replaced (The prices include labor):
Quantity: 1 Brake Pad SS8501X(item number) Location:R Price: $149.99
Quantity: 2 Brake Rotor 121.42088 (item number) Location:R Price: $232.60
Quantity: 1 Brake flush B5000 (item number) Price: $89.99
There is a column R/S that has numbers for each item mentioned above. What does it mean? The numbers for items listed are 1, 1 and 3.
My questions are:
Do I need to buy a pair of Brake pad for the rear?
Do I need to buy a pair of Rotor for the rear?
Can I replace the rotors by myself?
What is "brake flush"?
