Keep blowing O2 sensors, why??

1 week after I tuned my car one o2 sensor broke.
I replaced it, and within 24 hours the same one broke again.
It is the sensor towards the rear of the car coming down the Y pipe on the 5th GEN. Only that one burns out. Any ideas, these things aren't cheap. I don't know if my afc is goofing something up. ALL settings are correct (Hotwire, arrow up and right etc.)

my setup:
AE 5spd
V2 3.125
Stillen Y
Greddy evo
Custom CAI
Apex S-afc (tuned)
22,000 miles

Thanks for any advice

You can blow O2 sensors due to electrical surges. I think that AFC may be causing it.
Unburned carbon deposits due to higher lean condition|?? maybe
You could a defective part.

this sounds crazy check all your fuses and ensure none of them are broken. I just had a incident like that, I installed a Ypipe and the da mmmm fuse was broken making it seem as if I broke the mans O2 sensors. He got new sensors and the CEL light was still on, sure enough it was not the 02 sensors it was one of the fuses that had broken

Talk about CEL light. The wierdest thing is that after 4 months of CEL, code (P0420), the light gave up and went off by itself.
This is the error code thrown due to test pipe (instead of Cat)

When I replaced the sensor the light shut off and the car ran perfect for a while. I looked at the Y and one bolt is missing off the section closer to the rear coming down off the header. The O2 follows this about 4 inches after the missing bolt connection. Could be a possibility. Do you know which fuse? I'll check anyway. The AFC seems to work fine, monitoring function work and sensor checks read as they should. Thanks for the ideas.
