maxima's don't have the best 5speeds. I have a 1996 5spd and had the transmission changed at 80k miles ($2200 and 3 days labor) when a input shaft bearing went out. I now have 123k and no problems yet. No leaks (never had one) but i did put in Proline MTX tranmission fluid which i must say made a day to night difference. Best of luck.
First, tranny flush can only be done on an automatic. You can't do that with a 5-speed. Drain and refill is not the same as flushing. Second, I only did this because I didn't know how long its been since the fluid has been replaced. I went for synthetic mostly for thermal protection and it lasts way longer than petrolium types. Thermal protection is a must since its been hitting 106+ degrees here.

Does changing tranny fluid have a significant effect? Do you always have to flush the old stuff? Cost?

Is the JDM a stock replacement or a high performance unit? How much and where can you get one?
