Teeth on ring gear missing, why?

Took transmission in to have them replace the carrier bearing (like every other 5spd owner) They said everything looked good except the ring gear. Missing two teeth:

What would cause that to happen?? And having those two teeth (shop called them keys?) floating around didn't damage anything else? Thats hard to believe.

ouch.. that's ugly.

I can't really explain why, but most of the time those things aren't a huge issue to be in the tranny. you can see in this shot of my rebuild that there's a magnet in the bottom of the case to hold junk like that..

Engaging the starter while the car is already running can cause a problem like this...although I can't really tell that much about it from the picture (looks as if its broken over halfway down). Also if the starter isn't "meshing" fully with the flywheel it can cause unnecessary wear on it...
