What should I do with my car?

I purchased my 2005 Maxima in 2013 and liked it very much until it started hesitating and jerking when accelerating or slowing down. Felt like the gears were slipping. While driving up a steep mountain road the car will not go over 20 mph.
What should I do with my car?
Any suggestion will be appreciated.
