Clutch adjustment, or is it slipping?

My clutch seems to be slipping. Ive never had a 5-spd before, so I don't actually know what slipping is like. When I want to take off really fast, the car is great, but when I reach the higher revs, that's where the problem starts. It "shift", just like an automatic. It'll go up to 4-5k really fast, then go back down to 3kish. During that time I had constant pressure on the accelerator. I would've thought that a slipping clutch wouldn't allow the car to launch as well, but Im not noticing anything out of the ordinary for that... Could this be an adjustment or something? Or is this what commonly happens when a clutch slips?
