Brake vibrates -- need help

The brake on my 96SE has been vibrating and it is getting worse. The "frequency" of the vibrate varies depending on the speed so I assume it's either the rotor, wheel, or tires. A friend told me it may be that the rotors are worn un-evenly and have "waves". I visually inspected the rotors and pads and they appear fine.

This past weekend I rotated the front wheels to the rear to see if the vibration was caused by the wheel/tires. It became a little better until I took the car to the moutains. At some sharp turns it was vibrating so badly it felt like the ABS was kicking in. I doubt it was the ABS since I wasn't even cornering and braking very hard and the pavement was perfectly smooth and dry.

Any ideas??
