P0138 Code

So I have been getting this code here and there. It comes and goes, and never stays. When it does it pulled the P0138 code

P0138 - Downstream Oxygen Sensor Maximum Voltage fault

So does this mean it's bad?
Also what would this code affect? Anything?

Try Seafoam fuel cleaner before replacing sensor

I have 97,000 on my '03 and got a P1038 and before I replaced the sensor I tried Seafoam fuel cleaner (recommended by Advanced Auto) and sure enough it has enough cleaning agents in it to clear the sensors. I'd try this for $10.00 a bottle before the effort of replacing a sensor.
