when gas is pushed car wont pick up at times...plz check

Originally the rpms would just drop when i came to a stop. I replaced o2 sensor and cleaned throttle body, it seemed to stop for a week or so. But now it still does it but also when driving if i push the gas it wont go the rpms like stick and the car just is like blah...what could it be ? I have intake and full exhaust. Could there be a exhaust leak or is something loose...any ideas though ect. thanks
