transmission problem

I noticed that my friend's plymouth voyager will roll forward (like if the car were in neutral) when the car is put into reverse in a downhill slope. I know there has to be something wrong, since all cars I've seen will stop in reverse (without braking)on a hill. How serious is this problem??
On the freeway the car will sometimes hesitate to downshift making the car jerk. Otherwise, there are not other major issues.
