Yep, exactly what UMD_MaxSE said. Open loop will occur at WOT, will occur at all RPMs/throttle positions if the engine coolant temperature is below a certain amount (not sure the exact threshold temperature), and all the time if the O2 sensors are malfunctioning.
It might also enter open loop if there's a failure of the closed-loop algorithms (i.e. if the fuel system is running too inefficient that the ECU determined it cannot properly control the air/fuel mixture ratio... this happened in a family member's Mazda 626, ECU entered open-loop with an SES code indicating fuel mixture too lean)
