I've got the code "0315 Downstream O2 sensor high voltage"
(P0160 O2 sensor CKT no activity (bank 2 sensor 2)).
What this means? Which one is bank 2 sensor 2?

A and C have 3 wires. B and D have 4 wires.

I reseted the ECU but after about 50 miles came back again with the same code. I already replaced the sensor "D", about 500 miles ago.

Anyone with same car? I need some help here.
