O2 sensor code will not go away

I have a 98 GXE, and every several thousand miles I throw the same code 04 09. Which is the O2 sensor on the rear (closest to fire wall) branch of the Y-Pipe...someone please correct me if I am changing the wrong one. I have now replaced the sensor twice since I have had the car and I have only put about 40K on the car. Am I changing the proper O2 sensor for this code..and if so what in the heck is causing this problem? Also when I replace the sensor I have always used the Bosch OEM. Thanks for your help.
