Strange hesitation in 4th and 5th ONLY...look inside

I noticed that i am getting a strange hesitation in 4th and 5th (5MT).

It happens when I am cruising along, and usually around 60 mph, though it has happened above that speek. RPM are around 3K (give or take a few) in 4th and a little lower in 5th. If I go to accellerate, I get a strange result:

First, I can hear the engine noise build. After this, the RPM slowly climb a few hundred, BUT THEN they fluctuate a few times, by a hundred or so RPM. The same thing seems to happen in 5th, but the RPM are a bit different. I am trying to isolate speeds and engine RPM, btu it ifs hard to do that while watching traffic. Also, the highways here are 55 mph, so I have to watch for cops while experimenting.

I have several KNOWN problems in my car, which may or may not be contributing:
1. POSSIBLE exhaust leak between the B-pipe and the muffler, due to no gasket there. It is also POSSIBLE that the piping has a small leak.
2. CEL from an EVAP canister and valve problem.
3. CEL for KS, BUT every source (even the dealership service department) says that it is just an after-effect of the EVAP codes.

Sorry- the hesitation is at 3000 rpm in 4th or 5th- not @ 60 mph in both gears.

Mine hesitates too, (auto) at 38mph when transmission is in 4th gear (overdrive) , i step on the gas almost to the point of WOT, and nothing happens - no kickdown to 3rd - just slow build up of RPM's till 3500 -3800 than it moves but it feels so sluggish.

All the maintenance is up to speed, I have hacked air box . NO CODES ... Knock sensor has 15k on it.

I will try to clean the MAF sensor, and i will look at coolant temp sensor.
also the throttle position sensor is resonsible for providing ECU with inputs that relate to engine load and driver demand.

I will read over the links in the link there, btu one note i that I have all stock tranny components. Also, I am at about 108-109K.

I could use a tranny fluid change...

anyone shed some new light on this, i have the same EXACT problem, i twas really bad at first with a small intake, i replaced it with stock, helped a bit, then chagned my fuel filter and PCV valve, and it took about 80% of the problem away, but its ongoing
