PCV valve rubber housing ripped open

I went to clean the PCV valve and when I took off the valve the rubber gasket was barely hanging on the block (into which the valve snuggly fits in). I got half of the gasket hanging out and the other half probably went into the combustion chamber and vaporised.

How often does this happen to the rubber gasket? I have to get one from the dealer, provided he is stocking it...

i havent had it happen to me at all but i wouldnt worry about it too much, the part is only like 10 from the dealer, just pick another one up and replace it.

can i please get the instruction or diagram with how to replace/clean pcv valves?
i need to clean them soon.

The FAQ's stickies should have one. There was also a thread detailing the picture, look to the left of the air filter, there is a hose running to the block, kind off hidden but easily visible right next to the throttle spring assembly.

Can someone post a picture?
