Iittle did I know, i got my Check Engine Soon light back after I installed the O2 simulator to fix it. Right now, my codes are P0138 and P0158 (High voltage in banks 1 and 2)... what do i do now?
Check the unit. Is the green light on? If not, either your unit went bad or your wiring is faulty. If light is not on, re-check that your wirinig is still has good contacts. If contacts are still good and light is on, I have no more ideas.
Let me emphasize, the O2 simulator only provides the 0-1V which is not the only data seen by ECM. There is this high voltage, resistance and other codes associated with an O2 sensor.
I installed the O2 simulator and then got the resistance code and then said to myself, enough of these shortcuts, and went ahead and bought all 4 O2 sensors from www.ngk.com and installed it and called it a day. I have new O2s for the last 15K miles and there is no code related to O2, cats or anything with the exhaust system.
Now, my $0.02 is to just replace the O2s and move on!!!

are you running without cats?! If not, why u bypassing the O2's ?! Is the cat shot? Or you just trying to save money on the O2's?

The ECU is looking for a minimum of 0.6V to 1.0V during the secondary o2 sensor oscillations. The reason why you are getting those codes is that most likely the o2 simulator is defective and not outputting the proper minimum voltage the ECU is looking for.
Grab a multimeter and check the voltage output on the WHITE signal wire. Mine was outputting only 0.3V and hence was throwing those two codes. O2 simulator.com replaced mine for free
I had one of the rear O2's go bad on me. Since it is (not necessary, but) recommended to replace both at the same time, I opted for the cheaper route and replaced both O2 signals with one dual output O2 simulator for $25 + shipping.
Installed and have had no CEL light on for 6k miles since install. If this route works for you, why not go this route? MUCH cheaper than 2 new O2 sensors and your still not destroying the ozone layer since the primary O2 sensor takes care of that.
As I stated, check your wires to make sure they are still in contact w/each other as well as suggested above, perform some tests w/a multimeter if you have one handy.

Depending on where you got it from, they note in the instruction that you can temporarily connect the o2 sim to a 9V battery to monitor the output voltage prior to installation.
The defect rate is very low, just bad luck that's all.
