Bearing...a definite yes. The rubber spring insulator and rubber dust boot...probably yes because they can slowly wear out with use. Problem is you never really know if they need replacing until you take everything apart and look at them. Considering the overall cost & time to do the job, I'd go ahead and replace all the rubber parts so you have them on hand when you start the job and not have to make an extra trip with the car all torn up if you find you need them later on.
I'm replacing a crapton of parts next next weekend, got some konis, new springs, rsb, etc. I'm doing spring isolators/upper mounts, bearings and dust boots like you said. While you're down there you might as well replace your (quite possibly completely worn out) sway bar bushings with polyurethane ones, $12 for an endlink set and $12 for a frame set from the cattman group deal.
