92 Se, some problems

I have a 92 SE, that I just love, but I am worried about my baby. She is loaded, 5 speed, with 195k km. I was hopeing someone may recognise a problem.
1)An odd problem, that would bother others more than me.
It is a Candian vehicle and came with stock day time driving lights. Lately they have a mind of their own. One goes off , then they both go out then the left side high beam comes on. If I turn on the lights they work normally, and when I turn them off the high beam comes back on.
2) More serious, Very intermitantly idle rpm will drop down to almost a stall. Before it stalls it jumps right back up and returns to normal. I brought it in to a dealer, who looked at it for two days. Unfortunately problem does not happen often enough to diagnos.
3) Most disturbing,
This awful noise, rattles out of the engine especially when cold, for usually less than 2-3 seconds. Dealership tells me it is the VTC sprocket. It does not seem to affect the performance of the vehicle, but it sounds so bad I cringe every time it happens. A VTC unit was replaced under warrenty at 89 000 km. The noise made its presence 40 000 km later.
Any thoughts or similar occurances, I would love to hear them. I have moved to Michigan, where the Nissan Dealer is a GM dealer that carries Nissan. With the lower volume of SE's here I wonder if the mechanics get the same experience as another city with more Nissan on the road.

Noise for a few seconds after first starting is due to lack of oil. Better oil filters (like the Nissan one) have a valve that prevents the oil from flowing back down, and keeps enough oil in the passages to prevent the noise. I have a Purolator filter that also has this valve, and have no noise after starting. If changing the oil filter doesn't help, then suspect the VTC.
Unsteady idle seems to be a common problem. If your car didn't do this before, but does now, then something has changed. Perhaps a throttle body cleaning would help.
