Just change out the valve itself. No real need to change the grommet, but if you already bought it, then go ahead and change it out too. The valve is a bit of a ***** to get out, but just be patient and get a pair of vice grips. Work it back and forth. It'll come out after a bit of work.

The PCV valve is on the driver's side of the rear valve cover. You'll see a hose coming up from the valve cover, the PCV valve is connected to this hose. If you follow the hose, you'll notice it's being held in by a bracket and a 10mm bolt. Take out this bolt and it will allow the hose MUCH more room to move.

The hose and PCV valve just pull straight up out of the valve cover. Once you get it out reverse the order and done.
