Many auto that were converted to manuals have that same symptom.
I would try to adjust the default throttle stop and the IACV.
To adjust the default throttle stop, the car must be at operating temp, TPS disconnected. Idle speed should be set around 600 - 650. Reconnect TPS and adjust the IACV until you abtain the correct idle speed.

The whole process is found in the Haynes shop manual.

I've seen this problem before at my auto class. We were going to do an engine swap on a Toyota Carrola (I hope I spelled that right), we decided not to. The engine in it now had a problem almost like your problem, except its an automatic. We took the TB off, and cleaned it with brake cleaner. The butterfly in the TB was just covered in that bad stuff, you know what I'm talkin' about.

After we cleaned it all up, we noticed a fair amount of change in the idle control. We almost got into a wreck, well sorta. It was parked with the E-brake on, and suddenly, it slamed into drive(somebody must have slipped it into netral). We fixed it by removing the TP(throttle position) screw.

What I'm trying to get at, is it must be a problem with either the throttle body, throttle position sensor, and/or the the intake(somehow).

Sorry for the long story, I was trying to make a point. Also, sorry if this doesnt help you.
