I believe the "easiest" way to get one is to find a used Sentra Spec-V tranny and have it mounted to the 5.5 gen bellhousing. I say easiest because the Sentra trannies are far more numerous and cheaper; however, the fact of having to swap the bellhousings would inherently make the whole process more complicated than if you could find a 5.5 gen tranny with HLSD which are much rarer.
In either case, you best bet is going to be trying salvage parts suppliers.

I'm not sure. Whenever I looked up the subject, because for a while I was entertaining the 6MT w/ LSD on my 5th gen, it seemed like buying the Spec-V and having it mounted to the 5.5 bellhousing was the general course of action for the few who have done it. It could be that the 6th gen has LSD but the trans is not compatible with 5.5 gen or not mountable to the 5.5 gen bellhousing.

The fidanza flywheel is nice. You will find a lot of info on here about it. Just make sure you get a sprung clutch disc. When you go from the stock sprung flywheel / unsprung stock clutch disc setup to the unsprung fidanza flywheel, you need some "give" in your drivetrain somewhere. I personally matched a Valeo 350Z clutch to my fidanza flywheel. Although there were some minor fitment issues early on, I really like the way the combo feels and drives in my car. You will find alot more info on here about people using the OEM 350Z clutch from Nissan.
As for the tranny, I guess it depends on how much you are wanting to spend. I am also starting to be on the lookout for a similar LSD tranny. My synchros are starting to go bad. I will most likely be buying a new one for an 05 Maxima. The synchros are much better for 05-06 Maximas, but the HLSD was only offered in the 05. That transmission seems to be pretty rare and expensive on car-part.com. There are online Nissan parts dealers offering that transmission new for around $1800, and you might even be able to talk your local dealership into such a price.
