Car hesitates while im accelerating

Car hesitates while im accelerating
I have a 2000 se it has around 75000 miles im running 91 octane gas.
When I’m accelerating it seems like it hesitates then you can feel the power kick in again. Does anyone have any suggestions?? I dont know if something in the fuel sysytem is dirty or whats going on. also Sometimes when I leave a light it just falls on its face..it feels like i dont know how to drive a 5 speed!!!then it pickes back up again..
