Car fine at idle, but dies in gear??

This is about my dad's 2001. Lately he's been having a bunch of issues with it, with a bad MAF for the 2nd time in 3 months, and bad plugs. I changed both, and it greatly improved the car, but still has issues. When he starts it up, its fine, but when he puts it in gear, the RPM's go up and down and finally all the way down till the car dies. He tries it again and the car sorta accelerates weird the first 20 feet, then its fine. He pulls up to a light, lets off the gas, and the rpm's drop down to around 350-400, and then car feels like its about to die, and sometimes it does die. All the hoses seem to be hooked up to the intake fine, the throttle body is a little dirty but its nothign that should affect the car so greatly. The idle speed in Park is around 650-750 he said. Can somebody diagnose the problem here?

My IACV was in bad shape, so I had it replaced. When I did, the difference was night and day. I had mine replaced by my mechanic but it can be a DIY job.

I got mine at a dealer. I dont have a write up for you but all I can say is that its not gonna cost that much to have a mechanic do it. However, part price will vary.

Today i opened the hood and listened to the car, and it really sounds like an air leak below the throttle body area, i wasn't sure where because i have huge hands and couldnt get them in there to feel for a leak, but i'm 100% sure there is a leak somewhere there. Does this sound like an IACV issue still?

Also when accelerationg, above maybe 4k rpm, there's a whistling sound like the car is boosted, or like it has an air leak.
