P0140/P0160 w/Headers+O2sim

So, my car is throwing the codes P0140 & P0160 (B1S2 & B2S2 High Voltage Monitoring) and I have headers and a dual output O2 sim hooked up.
And I'm wondering, is this a fault in the connection of the O2 sim (I hooked up the wiring according to posts on the .Org, so maybe a loose ground), or my other concern is that the car was throwing P0140/P0160 before the headers/sim install, so maybe the signal coming off of an already faulty O2 sensor cannot be adjusted by the simulator to keep my SES off?
Also, the car seems to be idling rather rich on cold starts and leaning out at idle when it's warm, which I don't really remember it doing.

- When it was cold it used to idle ~14.0 AFR, now it is around 13.0. There hasn't been very much climate variation lately either.
- When the engine is warmed up it used to idle around ~15.0 AFR, sometimes it goes past 15.5+ now.
- Since my SES is on and it's throwing these codes I fear that it is actually affecting closed loop operation.
- However, these are supposed to be rear O2 sensors, so maybe my primaries are fouled?

Well that's what the codes are coming from.
But they're supposed to be secondary O2 sensors so unless the primary ones are not functioning (from my understanding) they do not affect the driveability of the car. That's all my SES is throwing, but I did previously have driveability issues with an old MAF without a code.
I'm gonna pull the codes, pull the intake off and clean the MAF, and check for leaks today.

Update.
Intake has no leaks, I haven't cleaned the MAF yet, Advance Auto only lets me rent tools that let me disassemble my car completely
Anyways, no lean codes, the OBDII scanner is saying the P0140/0160 is "no O2 activity detected" and my O2 simulator is blinking a green LED
Hooked my e-Manage back up and the car idles smoother now

I have the same problem, I put the O2 simulator two years ago and everything ran great. Now got these two codes, and simulator blinking green. I don't remember if it was a solid green or blinking green before. Does this mean my simulator isn't working, or something else is now bad?
