diagnosing transmission problems

All,m I'm new here and reviewing posts about the valve body etc. I just wanted to try to confirm what my transmisiosn issue is. This is a 2004 Maxima with 5sp auto.

Symptoms are: intermittently revs high and clunks into gear, clunks and jerks when decelerating to a stop, sometimes "wallows" doesnt move much when i'm accelerating from stop (often in dangerous intersections so that's fun...)

Can someone give me a diagnosis or direct me to a thread about this issue?
