One fuel Injector went out - what causes this

I just had one fuel Injector go out and it costsed a pretty penny to replace.

What can cause this to happen?
And what is the chances the others are getting ready to go out?

My car is a 95 Maxima SE. Can it be caused by me putting regular gas inside and not premium?

Are you sure its a fuel filter?? I only know of one fuel filter, and it doesn't go out, you just have to replace it every year or so.

Things fail, it happens.
Fuel injectors are not a common failure item on our cars, don't worry about the rest of them.
Your injectors cannot tell the differance between premimum & regular fuel.
They would inject water if you feed that through your fuel line as well, but please don't try that.
