a quickie write-up (at your own risk). the link Puppet provided is better.

1. take off wheel
2. take off caliper
3. take off rotor
4. bend back dust shield a bit
5. soak in wd-40 to loosen rust
6. hit the studs directly with a hammer. VERY hard until they pop out. its going to take a couple uber hard strikes. ear protection would be recommended as its a bit loud.
7. repeat for however many sides
8. install new studs and put the rotor only on
9. use the spacer and open ended lugs to press the studs in (inspect the studs to make sure)
10. make sure they are FULLY pressed in before reinstalling brakes
11. re-install brakes after everything is pressed in
12. pop wheel on
13. tighten lugs as much as you can, loosen then tighten to spec.
14. youre done.

 

For this step, get a mini sledge hammer or one with the heaviest head you can find, it'll make things much easier.
