Power Steering Belt Ripping up ? Whats going on here?

Basically, i was replacing all my belts. It was rather easy. Until, i started driving and the power steering/waterpump belt poped right off the pulleys. When i look at the belts it is all ripped up. So i thought that it wasnt aligned properly. So I remove all the belts again replace the belt and put the other belts back on. Thats when i thought everything was ok. it was driving ok and everything. Of course the moment i said it was ok. the belt ripped off again.

Someone please tell me what is going on here?
Any little bit of information counts.
HELP ME PLEASE!!!!

Did you tighten them all to spec? Did you remember to tighten the pulley nut inself as well as adjust the tensioners properly?

So the old belts worked fine and didn't shred. Only after you messed with them right? Tension way too tight?

Too loose. Belt won't shred. Just squeek. Too tight might make the belt shred. Especially how fast they got ruined. You just can't crank down on the tensioners and not consider how tight you are making them. It's easy to crank too much as the tensioner bolt doesn't get harder to turn as the tension rises. You have to at least estimate how tight the belt is by how far you can deflect the belt by hand.
