I bought brand new ones from the dealer last year. They were only about $20 - $25 each from what I can remember. You can change them without removing the fender by unbolting the door from the hinge, support the door with a floor jack or something, then swap the hinge out on the body and reattach the door. It may take a little tweaking to get it lined up right, but it's not too bad. One of the washers on each hinge was tack welded to the hinge, I guess that locks the position once the door was aligned at the factory. You can grind off the weld to remove and reuse the washer. I didn't weld it back on, and it's been on there for a year or so and hasn't moved.
If you're good at lining up body panels, then I'd say to take the fender off.. makes it much easier to get the door lined up properly, then you have to get the fender back on and lined up..
If you don't, it's a PITA to line up the door, but you don't have to worry about the fender. I recommend just puling the fender though.. less hassle, IMO.
