I have an 89 Nisaan Maxima, and the keyless entry on both of my doors went out about 2 years ago. I finally want to get it fixed.
Is this something that can be fixed or does it have to be replaced? Could it be a fuse of some sort?
Also if I have lost my keyless entry codes, where can I get them again?

You got a little metal thing with some numbers on it when you bought the car. It could be in your glovebox on the sticker. Mine also went out and I want to fix it but my parents don't. You can order a new keyless entry from a dealership for $60 i think or $80 (per door) but they will charge you a hell of a lot of money for labor. Like $200 in my area for you who knows.
The keyless entry also died on my 89 max just a few days ago. For the first thing I checked the fusebox left under the steering wheel. There was only one fuse out. I changed it and voila, the keyless entry came back to life. Check yours as well.
What you also might need is to rebuild actuators (front LH side - 5 wires one; and front RH side - 2 wires one).
After removing, open actuator and make sure that all machanical parts are moving (might be a little rusty), clean all the contacts inside ... lubricate ... close the box and test 'em. If successfull, open again and seal with clear (or whaterver) silicone. Now you are ready to mount it back.
PS: when removing links, be very carefull with plastic clips - it's very easy to brake 'em
