Idle Rough and Exhaust Puffing

I have a 95 SE 5-speed and it has run great since I have had it for almost a year. The car has a 128K miles and just started acting up the other day. It idles rough and when you are accelerating it is also rough and not smooth like it should be. Then if I go back to the exhaust at idle it puffs out exhaust. It doens't have any color to it and it smells normal. Its just not a constant flow. Its like something if maybe blocking the flow or I also thought one of the O2 sensors might be bad, but I figured I would throw a code for that. Also another thing I thought might be possible is that I got some bad gas. Any ideas? Thanks in advance.

Has the fuel filter ever been changed?

How rough is acceleration, does it 'pop' at WOT? If it does it might be a coil pack going bad maybe?

It doesn't pop at WOT. I changed the plugs and fuel filter today. It still seems like it is doing it. Also did a compression test and it looked good. Any other ideas? I might try and clean the TB tomorrow. Anything else I should try?
