Hesitates + feels bogged at times

Hesitates + feels bogged at times
I have over 110K on my '98, all maintenance has been done this summer, including a tune-up and a fuel induction cleaning done. The hesitation feels like the revs are pulsing as I accelerate b/w 2000-3000rpms and the bogging kicks in at a little over 2000rpm, what's the cause even after all this maintenace was done?

I had this problem twice:
Once was a failing temperature sensor that had not thrown a code yet, basically ecu thought it needed to run rich and warm the engine. didn't happen all the time, but would bog bad to the point of stalling at times.
Other time was a bad fuel injector, causing real rough running, not all the time, but at certain rpms. Felt like a cylinder was missing. Had not thrown a code either.
