Serious Driving issue, Need all the help i can get!

Well i've tried waiting for a while and fixing various things, or hoping the problem would even just fix itself, seeing as how it came out of no where. Its kind of hard to explain but I'll try my best and hopefully get some help from the ever so helpfull .org

Problem :: While driving, when im stopped and let go of the break, It will sit there...instead of moving like it should (either in drive or reverse). Sometimes it takes just a little gas and sometimes it takes even more for it to rev to about 1.5k/2k for the car to start moving, sometimes smooth, sometimes it shakes the engine till it gets moving...and City traffic is a b*tch! Almost feels like its manual but its automatic!(Almost feels like when ur givin it a little gas its waiting for the clutch to grab)

I just did a tuneup about 3 months ago..fuel filter, plugs, pcv, Just got my oil change. I know I've never replaced my egr but I'm unsure if this could cause whats happening. Also do have an intake leak from a screwed up maf adapter in the shortram form.

What could be the cause?!!!

Cant be the brakes, relatively new and the shaking / Weird Rev's it does wouldnt be related, i would be able to tell if it was from a stuck ebrake or a calibur. Oh yeah, sometimes when im driving when the gas pedal is steady goin at like 30 the Rev meter keeps goin up and down, but thats only every 1 in 5 drives most likely. Another thing is when its really cold it feels like it doesnt want to shift out at first and shifts really high // really hard...i let go of the gas when i feel the shift because im scared of how hard the shift is damage wise lol...intake leak maybe? mixed air getting in the TB or an egr problem ? Iunno !

Check the fluid level in your transmission while the engine and tranny are hot. If the level is ok, your transmission is probably shot.

Torque Converter? that would cause the tranny not to "grab" right? i dont know for sure, its all clutch for me, but good luck to you.
