Wierd High Idle

Sometimes my car has given me a high idle the whole time that the car is on. It idles at ~1600rpm and when I take the car out of park, it shifts really rough. Also 1st gear is kind of high to, when the high idle doesn't happen, it shifts around 2000rpm, but now it's at 3000rpm. Another thing is when I let go of the gas, the speed doesn't decrease, it just stays the same. This has happened a couple of times, but it really p!sses me off. I tried a grounding kit and a fuel injector cleaner and it does not help. Help please.

Oh yeah I forgot to mention, this happens even thought the car is warmed up, I haven't messed with the IAC, and I have a rear O2 sensor code.

Yeah kind of, espcially when I change to drive or reverse, it acts if it's going to peel out. I wonder what the hell is wrong.
