2004 Maxima rear: 11.50" x 0.35"
1995-2003 rear: 10.94" x .35"

I'm not sure if the rear disc has a larger diameter or not.

But I am sure it is a totally different design, with the parking brake built into the inside drum of the rear disc.

So it will not interchange easily, if at all.

Tried it, won't work. the '04 rear setup was one of the first systems I tried on the back of the 4th and 5th gens for the kits. It just won't fit in there when you look at the spacing around the wheel bearing and ABS sensor.

the '04 uses a drum type parking brake inside the rotor. it still uses a disc type caliper for the rear brake though. that seems to be the standard design on most cars now. they started them on the Z32 and have continued to phase out the integrated rear caliper/parking brake over the years. the 5th gen rear end is identical to the 4th gen, so they obviously didn't bother redesigning it to fit. Almost all of Nissan's model lineup now uses the drum/disc setup in the back now though.

I didn't read it anywhere.

The 5th and 6th GEN rear brakes are of totally different design.
I know it for a fact because I have worked on both types.

Nothing is impossible, but this swap is not worth the effort.
