Darn codes won't go away

I had the dealer check my SES codes on my 2000 Nissan Maxima SE (Fed spec) with 150K miles and it turns out that I'm STILL getting my O2 sensor codes (been going on for a several months now)....

P0135
P0141
P0154

and

P0300

I have a Cattman Y-pipe, and Megan Cat-back exhaust. I just want these darn codes to disappear but they keep coming back. I've already replaced the O2 sensors but the codes keep coming back. Is there something else that could be wrong (like a grounding issue)?

And what's causing the P0300? I replaced all 6 coils, spark plugs, and MAF from DaveB less than 1 year ago...

I've also changed out the crankshaft position sensor (POS and REF) but the codes are still there.
