Car stalls in the morning - Please read

Ok so it comes time for a smog check and I’m driving one day home from work my car just shuts off all dash lights come on -all lights remain on but car died. So I put on hazard lights and throw it in park I go to start the car and it has a hard time starting once started I was half mile from home just drove it there. Check engine light had come on due to this incident. Next few days same thing always hard to start then once started its fine. After a few days, less than a week, it just went away and car started and ran fine. I had smog done after codes cleared and passed. The codes that came on after the stall where p0021 and p0139. The p0139 is o2 sensor which is usually a fluke. The guy who ran the codes said the cam shaft CEL (p0021) usually makes car run rough and mine is not so don’t mind it for now.

Well now a month later its back but full effect and here is the pattern. Every morning when I start car is starts fine no struggle. I get about a mile or 2 down the road to work and car stalls. After that it starts with a struggle and then after that runs fine no stalls. It stalls out in the same spot every morning practically. Basically what I have noticed is if the car starts fine (usually in morning when cold - sometimes after sitting for while) then it will stall, if it has a rough start it won’t stall. Did notice my coolant was next to empty today and have brought it back to the max line with a 50/50 mix, any ideas? On another less specific note car idles at like 400 and I notice when revved up to say 2000 and above once pedal is released it stops at 1500 before lowering back down to idle? Not sure if that normal. Any help is appreciated.

edit: Also after it stalls and get to the rough start point it usually wont start if the a.c. is on.

Do the battery/brake lights come on at all? before stalling or any other symptoms?

Does the engine light always come on or flash before it dies/after it has died?

update

check engine light is on as of now i haven't cleared the codes since they came back. only once the car dies all the dash lights come on. brake oil every last one of them. once car is restarted no flashing but engine light is solid all other lights go off.

Can you elaborate on the conditions when the engine does die?

I can sure try..

I can sure try. Whether I’m driving or not it dies i just learned that this a.m. as moving my fiancé’s car and going back inside to hang her keys up came back it was dead with all lights in the dash on. Restarted, ruff start and it didn’t stall again. Usually I’m driving surface streets in rush hour traffic low speed because of traffic and it dies. A.C. on, or A.C. off radio on, or radio off doesn’t matter. If the car starts fine it will stall after about 1-2 miles if it has a rough start it will not stall? I don’t care so much about ruff start but I can’t have it stalling, I can only imagine if i was on the highway or something

edit: besides that car runs smooth and is also responsive besides the original noted quirks with the low idle, but car is not rough or anything else?

Just took a video of the car having trouble starting and how it stops at 1500 when letting off a rev. Will upload them and post a link when I get back to my computer in about an hour. Also unified the MAF sensor to check if that solved the start problem. No luck there.

edit : i recorded this on my iphone and it came out good but youtube made it crappy? cant really hear it trying to start. but anyways it demonstrates the sticking at 1500 after a rev pretty well http://www.youtube.com/watch?v=ervBmo_gh8U

edit : ok so i just drove home and guess what before i left i found out why my coolant was empty. found a leak will upload video soon. any idea if that could be the cause of all this somehow?

just finished installing new radiator and camshaft sensor. everything is fixed
