And suggestions for a cure. O2 sensor is showing good activity on scan tool .... disabled each injector individually and all appear to be working ... no apparent vacuum leaks. What am I missing ??

I had the same issue. I replaced both upstream O2 sensors and haven’t had that code pop up yet. Wasn’t fun getting the cab side O2 sensor. Have to remove the intake plenum.
I plan to replace the downstream O2 sensors at some point but those are hard to get to.

 

I replaced my front 2 O2 sensors as bank one upstream was bad. (Replace in pairs). Then code P2095 came up the next day. Meter ran front 124 but downstream bank one was 104. Just ordered both downstream sensors. Did a smoke test and no vacuum leak detected. Hope this may help you.
