This may actually be more useful. The red shows the line between manifold vacuum and the solenoid. I used the vacuum point on the top of the manifold for my vacuum source (sort of hidden by the blue hose) The blue shows to the solenoid from the rest of the system. Basically I just hooked up the 4th gens line where the blue line ends because there is a service port in that area. I had to move some hoses around a bit and extent the wire for the solenoid, but it results in a really clean, factory looking install.
Edit: Re-did my coloring (I failed kindergarten)

The 00 and 01 throttle bodies have the IACV mounted on the bottom. I believe coolant lines do run to the TB, just like with the 4th gen TB. The coolant lines attach to the two ports on the IACV.

Coolant Lines: I'm alittle confused. Are both the in and out coolant lines on the TB, or is the in on the TB and the out on the intake manifold? If both are on the TB why can you not leave it as is, if you have a 4th gen TB?
Question about evap. I believe the objects I have circled in this picture are the a couple of components of the evap system. Sorry, I don't know the technical names of these. In my pic, the silver thing gets vacuum from the manifold, then goes to the black circular object. The black object has 2 hoses, one looks like it's going to the EGR and one going to the canister. Would all I need is the EVAP canister purge volume control solenoid valve and I can get ride of these 2 components?
MaximumHP: In your first post here in the diagram, the blue hose goes straight to the canister and the orange getting vacuum from the manifold; is that right?

After looking at this closer with other members, I noticed my car is totally different because I have a 99. That is why my swap was so easy, I basically had what 2000's have. I'd have to look into this closer on the FSM to answer your question if no one else can help you.
You can keep the coolant loop through the TB if you want, most of us just get lazy and close the loop. If I have problems this winter, I will probably re-route mine through the TB also.
