HVAC Blower intermittent shut off

2001 Maxima 20th Anniversary Edition
Automatic heat and AC. (not sure what the actual name is)
I'm looking for advice on how to solve the following issue. The AC/heater blower motor will randomly stop working and then just as randomly turn back on. When it works it functions on all speeds.
The 15 amp fuses are good.
The relay clicks as expected when key is in the ON position.
When working the fan voltage is: Off: .5v 1: 5v 2: 8v 3: 10.8v 4: 12.4v
When not working voltage is: Off: .5v 1:.35v 2: .32v 3: .29v 4: .24v
Testing was done in the ECON mode.

looks like your on the right track. I know sometimes when blower motors start going out they may run intermittently or make noise.

My money is on the blower motor failing. $40 from Rockauto and the swap isn't too hard to do.

I recently changed the blower motor and everything functioned correctly for about a month.

you need to replace the part in your first picture. "fan control amplifier"
