I dont think they sell that cover separatly, i never saw a part number on it when i took my dash apart. I would sugguest you go to a junk yard or post a WTB in the Classifieds forum

Check with your local dealer. I purchased one there before. Cost me around $20.

The fog can be removed with plastic cleaner, I cleaned mine with Meguiar's but I am sure there are other plastic cleaners available. IE. Mothers, 3M....

I used 3M Fine Cut Rubbing Compound (I believe that's what it's called). It comes in a rather large bottle that should last you a lifetime. Picked it up at Pep Boys for $7 or $8.

Exactly. The "fog" or cloudiness is usually caused by glass cleaners containing ammonia. There was also a Technical Service Bulletin from Nissan concerning cloudiness on the clock face from alcohol-based cleaners. Apparently, the clear plastics Nissan uses can't hold up to harsh chemicals. I use EagleOne 20/20 auto glass cleaner on mine with no ill-effects. My clock face was really bad, so I took it out and polished the plastic with Blue Magic Plastic and Plexiglass cleaner. Worked excellent. Other reputable companies' products would probably be similar. A felt buffing wheel on a cordless drill helped speed things up (Caution: a 30,000 rpm Dremel will melt the plastic because of its speed!). A lot cheaper than a new on, and if you have it out, you may as well do it. I'll be doing mine next time I take my gauge cluster out.

As for the clock, you remove the center vent cluster. The center vents and the black (or woodgrain) plastic underneath (above the AC controls) is actually one piece. Using a screwdriver with cloth wrapped around it, gently pry up on the bottom center to "pop" the clips. Then I just pulled down and out from the top. You'll need to work with it gently, but it is made to just pop out. Once out, you can unplug the harness to the clock (if doing the "clock always bright" mod from Custommaxima.com, now would be a good time), the defroster and the hazards. The clock is removed from the plastic with the 2 phillips screws. The lens pops off. Easy to do, just take your time. I also used Mothers Back to Black on the black plastic to get it spotless while it was out (I removed my entire center console and trim and did them all at once. Looks brand new again!).
As for the gauge cluster, I can't remember off the top of my head. I'll be doing it maybe today if it stops raining.
Hope this helps.

Dave

...and keep the ArmorAll(or any other "protectant", for that matter) away from any clear plastic, too.
