MAJOR car problem!! Please Help

hey guys,
I have a big problem here. Yesterday afternoon my car was running 100% perfect, and all of sudden it started to jerk me forward. For example if i stop at a red light or stop sign, and then accelerate to go, i get whipped forward. It seems as if im in a 5 speed car and i don't shift properly. But i never worried too much about that. I just went outside now to get to my dentist appointment and as i was reversing out my driveway the car just starts jumping constantly. The RPM meter is jumping from 1000 to 0 over and over. I don't know what has happend. Please help guys. Thanks

you might have to clean your throttle body, if not its your tranny

im fearing its the transmission. Oh god help me. This is costly isn't it??

If it jumps from 0-1000, ther shouldnt be too much jerk, since idle is near 1k anyway. And if rpms are anywhere around or below 400, the car will shake and then stall.
