bogging/stalling after car is warm

I've been having an issue lately with my 1997 maxima. One day on my way to work I slowed down for a red light and when I stopped, my car shook for a second and then died out. It started right back up with no problem. Well now this is an ongoing issue and it's annoying as chit!

I do have an exhaust leak that's right after the headers where the two pipes merge together, idk if that makes a difference. After running my obd2 scanner I came up with codes p0400 p0150 p0141. Two of them are o2 sensors because 2 of my sensors are not hooked up (took exhaust system off 95 maxima, o2 sensor connectors arent the same). The other is saying there's something wrong with my EGR valve. Any help on this is much appreciated. Also, I was going to try disconnecting EGR valve vacuum tubes and plugging them for a day to see if a stuck EGR is the cause, but it's really difficult to get access to.

EDIT: I also forgot to add, I have been driving with the o2 sensors disconnected for maybe 1-2 months before this started to occur. This generally only happens when the car is warm up, I haven't had a problem when it's cold. It was also slightly bog down for a second randomly while I am driving at a steady speed. I've been putting it in neutral and slightly revving it when I stop to keep it from dieing out.

The EGR system is known to clog up. You may need to clean it out, do a search on here for detailed instructions. You need to get the o2's back on line.

I just fixed a P0400 code by pulling the EGR tube and cleaning it out. It's a common problem, and there are some excellent threads on it here. But I was running fine before that, and wouldn't think that would cause it to die.

If you put the car in N at a stop, will it still die? If not, try putting in new plugs (NGK only).

using the search feature, type in the following. MAF, TB, IACV, and EGR cleaning for dummies. do so and report back on results. and like NJmax said, get those 02 sensors back up, that's not helping anything.

I've got a pretty massive leak after my headers, so I am going to try and repair that first and see if that makes any difference. I'm really leaning towards the EGR valve being the issue though. It's a pain in the buns to get to it though!

Yes and no. Sometimes it will, sometimes it will not. Usually it gives signs of struggling to keep the RPM's up before it dies, so I know to hit the accelerator a bit.
