What the Hell??

I have a 2002 max, recently I started having a problem that I initially thought was in the transmission but now Im not so sure. I start the car put in drive and press the gas, it hesitates like it isnt getting fuel then it will jerk and take off. When it comes time to shift it will jerk, I let off the gas, it shifts then takes off again. At first I thought it was the transmission slipping but the engine doesnt rev before shifting, it just jerks then shifts. it does this through every gear change. Then once the car gets to about 63 or 64 mph it will start jerking again and wont go any faster and the engine doesnt rev the tach maxes out around 1400. Please help. Im putting it in the shop this weekend, but any advice on what it could be would be great. Thanks!
