Brutal Winter

This winters been brutal... I've been having issues with my 2000 Maxima auto when the temps are in the 20's and lower.. I've had to warm up the engine to normal operating temp, if not the car would stall.. The rpm's will bounce until engine is at it's operating temp.. The car threw a lean code a few times during the coldest of days... My idle is rough during these cold periods as well.. Now today the temps been in the 40's, I can start the car let it warm up with out having to press on the gas pedal to keep it from stalling... Most of this winter I've had to sit in the car pressing on the gas for about 10mins or so until the engine warms so the engine won't stall... I'm going to replace my valve cover gaskets as I know that their leaking and I can smell exhaust in the vents sometimes during idle... The car has almost 220,000 miles on it.... No codes for the crank position sensor, Maf sensor, Engine Coolant Temp Sensor, Idle Air Control Valve Sensor, Camshaft Sensor... The engine also seems to have a low end knock or valve tap when the temp outside it in the 30's and below.. If the temp is in the 40's and above I don't hear that knock or engine tap...

When was the last time you changed the oil, and what weight are you using?

Oil was changed a month ago... I've been using 5w 30

Doesn't seem like a common issue, my car has some slight hesitation when starting in the morning whenever it's been in the single digits, but once fired up, it's fine.
