Transmission funk

Hey y'all,

Having issues with the tranny in a 2012 SV. It does leak some fluid, but has never gotten too low. However, I just topped it off a few days ago and now having some real issues. It now seems to be slipping... I rechecked the fluid level and it is within the hash marks on the stick. I live in a hilly area and couldn't make it home so I parked in a lot, on an incline with the front heading downhill. Let it sit for a few hours, got in it and drove it home with no issues. It seems to be very intermittent, but once it starts, the car won't rev above 2k rpm. Is it a fluid issue? Is it toast? need some guidance? BTW, did a DIY transmission swap about two years ago, no issues until now. Thanks!

262k, not sure how many actual miles there are on the transmission. This only seems to happen when the tranny fluid gets topped off. Forgot to mention the last time i had the oil changed, they topped off the fluid, same "slippage" issues followed.
