You should Power Wash the hell out of your floor matts, @ one of those DIY car washes. Then take Orange Clean/409 ect. & a damp rag or sponge then spray the damp rag or sponge alot with it & scrub the headliner everywhere. This will take away any odor & most stains.
This will also help your smoke situation. I would scrub all the headliner & also scrub all the dash pieces, seatbelts, seats, handles everything basically like above. This will help get most odor out. Then Apply lots of Lexol Leather Cleaner to the seats & let it sit in the sun for a day. That way it will soak in, & so will the scent.

Acutally what I do is buy a box of Bounce Dryer sheets and put one under each seat every week when I finish washing it. It keeps the car smelling fresh.
