Tire Problem: Symptoms and causes of ripped belt?...

Had an alignment done on Saturday (2 weeks after AGX install). The alignment went fine but when the tech came back he said the vibration I had complained of was from a ripped belt in the tire. This is the second bad experience I had with these tires, as after 3k miles I had a bubble develop in one. Now they have only 13k miles and a ripped belt. So my questions are as follows:

1. What can cause a belt to rip?

2. Would sever vibration of the steering wheel be a symptom?

3. Is there any way for me to visibly check to see if they are correct?

I already replaced the tires with my stock steelies tonight, and tomorrow when I take it for a spin I will see if the vibration is coming from the tires or not. Thanks for any help in advance,

Yeah Yokohama Avid H4s and come tomorrow I am going to call Yokohama and demand some new tires. Hopefully they can get me something other than Avids.
