Help with Levoc install

Do we use the small round adapter, large or neither.

Don't recall using any adapter.
Oh, I can only get the **** to fit on there
w/ just one screw.

I didn't use any adapters, and yes, you can use only one screw. Put the screw in the top hole, the **** will be shorter this way.
